I'm looking into Straight Talk Home Phone for a family member - basically you buy a device that connects to the cell network and allows you to hook up a regular landline or cordless phone.

When I put in my ZIP code it only offers me devices that operate on the CDMA network - does anyone know if an AT&T version is available, or if another carrier offers this service for ~$20 a month or less?

Only thing I see is from AT&T. $20 a month and free device if you have a 2 year contract. $45 activation fee.

Buy a used box off eBay for about 20 dollars and plug in a SIM card from any AT&T mvno. I use the 10 dollar for 90 days in my home box on H2O wireless

You can grab a unlimited talk and text for 20 from Red Pocket or Airvoice and use the SIM in it or if you need less minutes look at the Red Pocket thread in the general forum

Any specific model you'd recommend? I remember looking at a decent AT&T branded one (ZTE I think) but couldn't tell for sure if it was locked.

I have the ZTE wf720. Can buy an unlock code off eBay for a couple dollars if needed. For an AT&T MVNO unlocking is not needed.

Hope this helps
