Looks like its a lot of band 4 (15x15?) and some band 2 and a bit of band 12. Might be relying on band 4 mostly, and need of backhaul. Looks like you're also at the state border... Not sure how that affects things

So band 4 in Lake and Porter counties, IN is 20 MHz. So what happened was in the middle of that site gap you see along my route I connected to a site in IL, probably a few miles away at least and was on 15 MHz B4 for that .3 Mbps test.
For this side of the state line B4 is 20 MHz, B2 10 MHz and of course B12 5 MHz.
I find T-Mobile supplies great backhaul to its sites, the issue seems to be airlink congestion and site tuning. If I take a 4x4 device close up and directly in LoS to a sector I can pull over 200 on B4 alone.
